Our services also reach into the community where we offer a team approach of support for people who have a long term disability to independently engage in their own community. Our goal is to identify and remove barriers for these people so they can progressively gain confidence to become independent in their chosen activities.
We also provides support to clients in the community focusing on goal achievement and personal development within the individual’s own environment. Support is functional, inclusive and based on real life activities. Access to this support is via referral through ACC and the Ministry of Health.
Supports provided in the community are provided by specialist teams and focus on:
- Reducing the client’s barriers to participation
- Increasing the client’s independence, meaningful engagement and participation in the local community
- Increasing the client’s achievement of personal, community and employment goals to engage in life roles.
This service is available at our Naenae base only.
